Must provide a copy of the player's birth certificate by either attaching a digital copy of it to this form, or at a LIVE Registration to confirm your child's enrollment with the league.

*Coaches Pitch (7-8) Age as of May 1, 2024
* Little League (9-12) Age as of May 1, 2024

Online registration forms will need birth certificates verified, either by submitting a digital copy on the following form, or by bringing it to a LIVE registration. To be able to play this season, both the birth certificate and the payment need to be verified. Please check to see that your child's registration is in order BEFORE the deadline.
